Registration Form

At this time, we cannot offer online registration for the annual meeting, but we can provide the forms that you will need to mail to the address below. Please note that the the fees for registration will increase after March 15, so please send your registration forms before then.

There are two versions of the registration form: a regular pdf document and an interactive pdf document. You can fill out the regular version by hand or with a typewriter. The interactive version allows you to use your computer to fill out the form.Whichever one you choose, you will need to print out the form and mail it to the address below.

Follow these steps to download the form.

1. Verify that you have a copy of the free Acrobat Reader software from Adobe. If you do not have it, you may download it from Adobe's web site.
2. Click on one of the following links to download the interactive version or the regular version of the form.
3. Open the form using Acrobat Reader. If you elect to use the regular version, print it out and complete the form by hand or with a typewriter, then proceed to step 6. If you choose the interactive version, proceed to the step 4.
4. Use your mouse or the tab key to move around the document and fill in the appropriate fields. (The check boxes may be filled in with a click of the mouse or by pressing the RETURN key.). The calculation for the total cost will be made for you automatically.
5. Print the form immediately after you have completed it. If you close the document, you will lose all of your work, so be certain that you have filled out the form accurately and completely.
6. Put the form in an envelope, enclose your check (CAMWS cannot accept credit cards), and mail it to the following address:
